Unico museo degli Stati Uniti interamente dedicato alla ritrattistica, ha aperto i battenti nel 1968 all'interno dell'Old Patent Office Building — un National Historic Landmark avviato nel 1836, condiviso con lo Smithsonian American Art Museum. Al secondo piano, l'esposizione permanente "America's Presidents" accoglie l'unica serie completa di ritratti presidenziali al di fuori della White House, imperniata sul celebre Lansdowne portrait di George Washington dipinto da Gilbert Stuart e sull'opera del 2018 di Kehinde Wiley raffigurante Barack Obama. Nel resto dell'edificio, la collezione spazia dai soggetti di epoca coloniale alle commissioni contemporanee, mentre il Robert and Arlene Kogod Courtyard, con la sua spettacolare copertura a vetri, funge da salotto urbano ad accesso libero tra una galleria e l'altra.
